NUR 253B - FNP III: Care of the Adult and Gerontological Patient and their Families
Semester Hours: 4
This is the third of four clinical courses, which provides a comprehensive foundation of multisystem cardiology, pulmonary, renal, gastrointestinal, and endocrine clinical problems, and a precepted primary care clinical practicum. Students will have precepted clinical placements in a variety of primary care settings. Students will also make home visits to patients seen in the primary care setting as they concentrate on transitions of care and coordination of care. The home visit model engages patients and their communities in a health partnership, improving health outcomes and decreasing readmissions. The purpose of this practicum is to follow a systems approach to further refine skills in differential diagnoses and identification of less commonly seen acute and chronic illnesses in the young adult, adult, and geriatric population. A strong focus will be on assessing, diagnosing, and managing culturally diverse patients with acute and chronic complex illnesses. Learning in this course is a collaborative process in an interprofessional learning environment providing quality, holistic, scientifically sound, and patient-centered care. Clinical placements will reflect the community needs, many of which are from an underserved population.
